How do I enroll? New patient register here.
- Check with insurance for Telehealth coverage or Click here for self pay rates
- Have a active Patient Portal account with the practice
- Have a High speed internet connection
- Have a recent (within 1 month of appointment) vitals taken either with PCP or Home device (Weight, Blood Pressure and Pulse Rate)
- Have a device with Audio – Video capability. (Computer with camera and microphone or Tablet or phone)
- Sign the telehealth consent form and fax to 866 510 2730 or email to endodiab911@gmail.com. Telehealth Consent Form
How to make an appointment?
- Make an appointment online with Dr. Raju, MD or Hadia Jaffery, PA-C or call 314-843-4848 to make appointment.
- For same day appointments call office.
- Make your selfpay/copay/coinsurance/deductible payments here and they are due PRIOR to appointment.
- Its your responsibility to fill all the forms required either online through patient portal or download the forms from the website and email/fax to us.
